Instructions for Swab Collection Instructions for hair sample collection for Confidential Paternity Test (2 people):

General Notes:

* Wash your hand throughfully with soap prior to collection;
* Read the instructions carefully and make sure that you understand every step.

1. Prepare:

* A labelled sample envelope or Ziploc (1 per person)
* 1 shipment (return) envelope

2. Label the first envelope or Ziploc bag with an ID, ethnicity, date of birth, and its relation to other samples

(ex., "SAMPLE A - ALLEGED FATHER")

3. Using a kit swab or a clean, unused, Q-Tip (cut in half) collect the sample as follows:

* Insert polyester tip of the swab into your mouth
* Press the tip against your inner cheek. To ensure that the enough pressure is applied, you should feel the swab on the outer part of the cheek
* Roll the swab 10 times up-and-down the side of your cheek
* Repeat the same procedure on the other side of your mouth with the same swab
* Holding the swab, allow it to air dry for about 30 seconds before placing it in the corresponding sample envelope or Ziploc
* Repeat the process with the remaining swab

4. Seal the sample envelope or Ziploc

Note: Please, DO NOT LICK THE ENVELOPE, USE WATER

5. Repeat steps 2-5 for all other participants.

7. Fill-out the submission form or include a note with your contact information and details about the case.

8. Mark the shipment envelope with Forensic Genetic Laboratory's address:

9. Place items in return envelope and seal. Attach a stamp with the proper value to the shipment envelope and mail.

1. Prepare:

* 2 clear zip lock bags (1 per person);
* 1 shipment envelope.

2. Mark the first zip lock bag with an ID and its relation to other samples (ex., "SAMPLE A - ALLEGED FATHER").

Note: Please, DO NOT LICK THE ENVELOPE!

3. Pluck 5-10 hair strands from the head

4. Make sure that the plucked hair strand contains the root since that is the site of DNA extraction

5. Place the plucked hair into the marked zip lock bag.

6. Follow the same collection steps (2-5) for the next person.

7. Fill-out the submission form or include a note with you contact information and details about the case.

8. Mark the shipment envelope with Forensic Genetics Laboratory's address:

9. Attach a stamp with the proper value to the shipment envelope and mail.

10. In the final shipment envelope please include:

* the collected samples;
* the submission form;
* payment (if the test is not purchased online)
